Full Job Description The Role:
The East Longmeadow Public Schools is seeking a compassionate, collaborative, and highly organized Family Outreach & Care Coordinator to support students and families experiencing challenges with school attendance, engagement, and access to school-based supports. Working closely with district school nurses, administrators, the METCO Director, counselors, social workers, and other school personnel, this position will help identify barriers to attendance and support students with complex medical or health-related needs who may require additional coordination and resources to successfully access their education. The coordinator will establish positive relationships with students and families, conduct outreach and follow-up, connect families with appropriate school and community resources, and assist with attendance intervention and care coordination efforts. The position will work collaboratively with the school nursing team to help ensure that students with specialized health needs and their families receive appropriate support and coordination. Strong communication, organization, confidentiality, cultural responsiveness, and the ability to work effectively as part of a multidisciplinary team are essential. ELPS is committed to creating a diverse and inclusive work environment where employees feel valued, respected and are empowered to be their unique selves. We believe that our administration and District staff should reflect the increasingly diverse community we serve, and that we should be intentional about elevating diverse voices and perspectives as an integral part of our work with students. Together, we can continue to build an inclusive work culture that centers equity, encourages empathy, supports belonging, and embraces cultural diversity as an important asset to the overall success of our schools.
Why You Should Join Our Team:
East Longmeadow Public School District is committed to cultivating an environment where diverse perspectives and backgrounds are embraced, acknowledging that a team reflecting diversity of race, ethnicity, national origin, religion, age, gender, sexual orientation, gender identity, disability status and veteran status allows us to serve our communities better. To that end, we welcome and strongly encourage applicants to bring their authentic selves when considering employment opportunities within our school district.
