Service Coordinator Jivan Adult Day Health Center Attleboro Falls, MA Job Details Part-time From $20 an hour 4 hours ago Qualifications Teamwork Writing skills Bachelor's degree Human Services Social Work Full Job Description J Job Summary The Service Coordinator works collaboratively with the interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate the services needed to support each participant. Service coordination is provided throughout the participant's involvement in the program, including enrollment, ongoing care, significant changes in condition, and discharge planning. Responsibilities and Duties The Service Coordinator is responsible for: Participating in the completion of comprehensive assessments and individualized plans of care. Coordinating participants' medically related physical, mental health, behavioral health, and social service needs. Documenting service coordination activities in each participant's health record at the commencement of services, following a significant change, and at least once every 90 days. Documentation must accurately reflect the participant's current needs and services. Monitoring and evaluating services to ensure they continue to meet each participant's needs. Informing participants and their families of available community services and making appropriate referrals. Assisting participants with personal, social, family, or adjustment concerns that may affect their participation in the program. Referring participants and families to appropriate community resources when specialized counseling or additional support is needed. Coordinating services with outside healthcare providers and community agencies when another organization is not already serving as the participant's primary service coordinator. Participating in discharge planning and coordinating referrals and services needed to support a safe transition from the program. Communicating relevant changes and concerns to the interdisciplinary team and participating in care plan reviews and team meetings. Qualifications and Skills The Service Coordinator must meet one of the following qualification requirements: Be appropriately licensed as a social worker; or Hold at least a bachelor's degree in human services, social work, psychology, sociology, or another related field from an accredited college or university and have at least one year of relevant, recent professional experience working with adults. The successful candidate must also demonstrate strong communication, documentation, organization, problem-solving, and service coordination skills. Experience working with older adults, adults with disabilities, or participants in an adult day health setting is preferred.
