Job Description

Care Coordinator - PRN Positions Available Reports to: Executive Director

POSITION SUMMARY

The Care Coordinator is a primary member of the Serenity Village care team that provides comfort and companionship to guests, their loved ones, and families at end of life in a relaxed homelike setting with a focus on comforting companionship. Care Coordinators also assist with all other duties necessary to successfully operate Serenity Village. This is a part-time, non-exempt position, hired by and directly accountable to the Executive Director. Bi-weekly pay, 36 hours/ week, 72 hours/pay period. Shifts are typically 12hours in length. Hours may be variable depending upon patient census and may include on-call, evening and weekend shifts.

KEY RESPONSIBILITIES

Responsibilities include the following, and other duties as assigned: Work in coordination with the guest's family and loved ones to provide comfort, support and care. Complete accurate documentation of guest interaction during each shift and provide shift hand off report between Care Coordinators. Assist guests with personal care that could include toileting, bathing, oral care, dressing, feeding, repositioning. Help prepare for receiving new guests and the exiting of guests after their passing. Provide support to guests' loved ones and family. Help train and work in coordination with staff members and volunteers. Maintain the Serenity Village facility by completing daily tasks that may include meal prep, laundry, janitorial and other housekeeping duties. Greet any who comes to the Serenity Village and assist as needed. Notify the Executive Director of all appropriate guest, facility or process concerns.

QUALIFICATIONS AND REQUIREMENTS

High school diploma required. Medical certification preferred. Must be physically, emotionally, and mentally fit to perform all duties and care for guests. Must pass background check. Must be able to lift 50 pounds. Complete all required internal trainings, certifications, and updates. Must be dependable, responsible, and have good communication skills. Must have a positive attitude, ability to work with others in a team setting, and add to the caring, positive culture of Serenity Village.

NOTE:

Serenity Village is a smoke free environment including, but not limited to, all tobacco products, vapes and marijuana.
