Overview The Resident Care Coordinator plays a vital role in ensuring the delivery of comprehensive, compassionate, and high-quality care to residents within a long-term care or assisted living facility. As a key member of our healthcare team, the Resident Care Coordinator will facilitate effective communication among staff, residents, and families to promote optimal health outcomes and resident satisfaction. Duties Coordinate and oversee resident care plans, ensuring they meet individual needs while complying with state healthcare regulations and facility policies. Conduct comprehensive assessments of residents' physical and mental health status, utilizing medical terminology and diagnostic evaluation techniques. Supervise nursing staff and care team members, providing guidance on clinical procedures such as IV infusion, catheter care, tube feeding, and wound management. Ensure accurate medical documentation using EMR (Electronic Medical Records) systems like Cerner or Athenahealth; maintain compliance with HIPAA standards. Collaborate with physicians, therapists, and family members to develop discharge planning strategies and coordinate referrals for specialized services such as dementia care or behavioral health. Monitor vital signs, medication administration, and other patient care activities to uphold nursing policy compliance and safety standards. Lead quality assurance initiatives related to infection control, acute pain management, and long-term care regulations; facilitate staff training on aseptic technique and basic life support. Manage case management processes including intake assessments, utilization review, and documentation of medical records for regulatory audits. Requirements Valid nursing license (RN) with current registration; experience working within long-term care or assisted living environments preferred. Demonstrated expertise in critical care settings such as ICU or Level I Trauma Centers; familiarity with

ICD-9/ICD-10

coding is essential. Extensive knowledge of hospital medicine, emergency medicine protocols, and acute care procedures including ventilator management and cardiac catheterization. Proven supervisory experience managing multidisciplinary teams in fast-paced healthcare settings; strong managerial communication skills required. Proficiency in EMR/EHR systems such as Cerner or eClinicalWorks; familiarity with health coaching, triage procedures, and medical imaging is advantageous. Strong understanding of physical examinations, specimen collection/processing, phlebotomy, and injection administration techniques. Knowledge of state healthcare regulations governing long-term care facilities; experience working with individuals with disabilities or developmental delays is highly desirable. Excellent organizational skills with attention to detail in medical documentation, discharge planning, and medication management.
