Responsibilities Act as the main liaison between patients, families, healthcare providers, and community resources to coordinate care effectively. Conduct intake assessments to gather essential patient information and determine appropriate services. Tracking care plans Completing quarterly reviews and reports Assisting with file organization of documentation management Review and documents to ensure all compliance is our matching up with EVV system notes Maintain accurate documentation within Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems while adhering to HIPAA regulations. Support utilization review and utilization management processes to ensure appropriate level of care and resource allocation.

Pay:

$16.25 - $18.00 per hour
