Job Description

Job Overview We are seeking a dynamic and compassionate Client Care Coordinator to join our dedicated healthcare team. In this vital role, you will serve as the primary point of contact for patients and their families, ensuring seamless communication, comprehensive Non-medical home care planning, and efficient coordination of care services. Your expertise will help foster positive patient experiences, facilitate effective case management, This position offers an exciting opportunity to make a meaningful impact in a fast-paced, patient-centered environment. Responsibilities Act as the main liaison between patients, families, healthcare providers, and community resources to coordinate care effectively. Conduct intake assessments to gather essential patient information and determine appropriate services. Tracking care plans Completing quarterly reviews and reports Assisting with file organization of documentation management Review and documents to ensure all compliance is our matching up with EVV system notes Maintain accurate documentation within Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems while adhering to HIPAA regulations. Support utilization review and utilization management processes to ensure appropriate level of care and resource allocation.
Pay:
$16.25 - $18.00 per hour
