About Us Emerest Health of New Jersey is a licensed home care agency providing non-skilled home care services throughout New Jersey. We work closely with patients, families, managed care organizations, and community partners to coordinate reliable care in the home. We are seeking a Bilingual English/Spanish Intake & Referral Relations Coordinator to join our Linden office. The Intake & Referrals Coordinator is the first point of contact for our clients and their families. This is a unique opportunity to serve as the face of our agency and make a meaningful impact to patients in need across New Jersey. Position Summary The Intake & Referral Relations Coordinator manages new patient referrals from initial inquiry through approval and start of care. This position communicates regularly with patients, families, HMO case managers, and referral partners while coordinating internally with our Nursing, Staffing, and Care Coordination teams. The role also supports agency growth through lead follow-up, case manager outreach, and development of referral relationships. What We're Looking For We are looking for someone who is organized, proactive, personable, and persistent; a problem-solver who takes initiative and is passionate about helping others. The right candidate is able to work independently, communicates confidently by phone and email, is comfortable making persistent outreach calls, and enjoys developing professional relationships. Responsibilities Receive, process, and track new patient referrals from initial intake through start of care. Verify Medicaid/insurance eligibility and collect required patient information and documentation. Explain the home care process to patients and families and provide updates throughout intake. Submit authorization requests to HMOs and follow up on assessments, approvals, authorizations, and other outstanding items. Troubleshoot authorization request issues through persistent communication with HMOs and patients. Maintain regular communication with HMO case managers regarding referrals, staffing progress, and case status. Coordinate with Nursing, Staffing, and Care Coordination to ensure a smooth transition to services. Maintain accurate referral records and patient information in agency systems, including HHAeXchange (HHAX). Respond to prospective patient inquiries from phone calls, website submissions, and marketing campaigns. Build and maintain professional relationships with existing and prospective referral partners to increase referral volume. Maintain accurate HMO/MCO case manager and referral contact information.
Qualifications Required:
Fluent in English and Spanish Strong verbal and written communication skills Excellent organization, attention to detail, and follow-through Comfortable making frequent outbound calls and following up consistently Ability to manage multiple priorities in a fast-paced office environment Proficiency with computers, email, and web-based systems High school diploma or equivalent
Preferred:
