Job Description

Social Work Manager Care Coordination The Social Work Manager will: As a member of the Care Coordination administrative team, is responsible for management of the daily operations and supervision of the Social Work staff. Responsible for integrating Social Work services into the Departmental service and the organization's overall system for intervention re: psychosocial, financial, and behavioral health aspects of care, progression of care, reimbursement of care, discharge planning, and transitions in care. Manages the operations of the Hospital's ambulance transportation contract. Key leader in behavioral health and ethical issues. Works directly and collaboratively with the Manager of Care Coordination Nursing to assure excellent level of service and to achieve Departmental goals and objectives.
Requirements:
1. Master's degree from an accredited school of social work. 2. Licensure by the State of NJ as a licensed clinical social worker (LCSW). 3. NPI and Medical Non-Billing Provider Number are required; the application should be submitted to the State within the first 30 days of employment. 4.
Clinical Supervision Certificate:
NJ Board of Social Work Examiners requirement to provide clinical supervision for LSW or within one year of hire. 5. Certified EARC Trainer-Department of Health and Senior Services or within six months of hire. 6. At least five (5) years of social work and discharge planning experience in a clinical setting; acute care hospital preferred. 7. Managerial experience preferred. 8. Knowledgeable in psychosocial assessment, case management, discharge planning/transitions in care, and associated regulations/compliance requirements. 9. Excellent written and oral communication and interpersonal skills as well as strong planning and organizational skills. 10. Strong critical thinking and analytical abilities required for addressing and resolving complex patient situations.
Salary Range:
105,615.00 - 169,023.00 USD We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials. The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
