Discharge Planner Location 1600 N. Main Street, Lovington, NM, 88260, United States Employee Type Full-Time Contact information Name Kellie Garner Phone 575-396-6611 Ext 1966 Email careersnlgh Description Provides case management services to patients by monitoring the provision of health care services to patients in order to ensur continuity, proper utilization of resources and maximization of clinical outcomes. Serves as Swing Bed Coordinator, Utilization Review Coordinator, Social Services/ Discharge Planner. Works in a close collaborative role with physicians and health care professionals who are providing care and services. Worksclosely with patients and their families. Has frequent contact with office staff of physicians, pre-hospital and post-hospital care providers, community agencies and other professional health care groups. Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans. Ensures that assessment and treatment on correct patient and the correct process is being performed. Takes and monitors patients' vital signs. Prepares and administers (orally, subcutaneously, through and IV) and records prescribed medications and reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse. Teaches patients and significant others how to manage their post-treatment home care needs, self-administration of medication and rehabilitation. Monitors patients' physical and emotional well-being throughout the care continuum. Initiates patient education plan, as prescribed by physician.   • Hospital/Community based Expert Decision making Experience Four years as registered nurse in an acute care facility, supervisory and case management experience preferred. Education Graduate of an accredited School of Professional Nursing. Licensure/Certification
