Overview We are seeking a dedicated and organized Home Care Coordinator to join our homecare team. The ideal candidate will serve as a vital link between patients, families, and healthcare providers, ensuring seamless coordination of home care services. Candidates with experience in medical offices, familiarity with medical terminology, and office administration skills are highly encouraged to apply. Responsibilities Coordinate and schedule home care services Communicate with patients and caregivers daily Ensure visits are properly staffed and confirmed Follow up on call-outs and scheduling changes Maintain accurate documentation in agency systems Requirements Home care scheduling experience Strong communication and organizational skills Ability to multitask in a fast-paced environment Bilingual (English/Creole)
