Working on-site at a major medical center in Bronx NY, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services.

Provide education to patients, family members, and referral sources regarding the Hospice benefit and range of services. Collaborate with referral sources from a variety of Health care practice areas to coordinate Home and in-patient hospice services.

Qualifications:

Licensed with current Registration to practice as Registered Professional Nurse in NYS is required. Bachelor's Degree preferred. Must have a minimum of two years experience in Hospice nursing with proficiency in physical assessment and interviewing skills.
