Position Summary The Youth Care Coordinator establishes supportive relationships with youth ages 5-21 and their families to improve overall health outcomes and social functioning through comprehensive care coordination. Essential Responsibilities Demonstrate knowledge of childhood psychiatric disorders, chronic medical conditions, and complex trauma. Conduct community outreach with youth experiencing medical, behavioral health, and substance use disorders. Complete CANS-NY, Comprehensive Assessments, and Complex Trauma Assessments. Develop and revise individualized care plans. Maintain monthly telephonic and face-to-face contact. Advocate for members to obtain medical, behavioral health, and community resources. Coordinate medical care, insurance coverage, and treatment providers. Collaborate with multidisciplinary teams. Provide crisis intervention. Maintain complete Electronic Medical Record documentation. Coordinate enrollments, closures, and caseload updates. Complete monthly Medicaid documentation and billing requirements. Participate in care management meetings. Coordinate office and field schedules. Serve as a resource regarding psychosocial, medical, and substance use issues. Travel throughout Suffolk County and, when necessary, Nassau County. Attend required Health Home trainings. Participate in supervision and High-Risk Review meetings. Maintain agency vehicles, laptop, cell phone, and assigned equipment. Maintain access to an insured personal vehicle and submit mileage reports. Qualifications Bachelor's Degree with two years of related experience; or Registered Nurse with two years of experience; or Master's Degree with one year of experience. CANS-NY Certification required. Valid Driver's License. Personal vehicle required.

Pay:

$25.00 - $35.00 per hour
