Healing Steps Recovery LLC is seeking a dependable, organized, and youth-focused Care Coordinator to assist with the coordination and delivery of services for children participating in behavioral health and youth programming. The Care Coordinator will work with children, families, case management staff, transportation staff, and program leadership to help ensure services are provided safely, consistently, and professionally.
QUALIFICATIONS
Valid driver's license and ability to meet agency transportation requirements. Dependable and able to work consistently during scheduled program hours. Strong communication, organization, and interpersonal skills. Ability to complete required documentation accurately and on time. Experience working with children is not mandatory, but childcare, behavioral health, education, mentoring, case management, or youth-program experience is a plus.
PRIMARY RESPONSIBILITIES
Youth Supervision & Care Coordination:
Provide appropriate supervision and support to assigned children, including monitoring participation, behavior, peer interactions, transitions, and engagement.
Daily Progress Notes:
Complete accurate and timely daily progress notes for supervised children, documenting relevant services, interventions, participation, behaviors, progress, barriers, and responses.
TBS Caseload:
Provide and document Therapeutic Behavioral Services (TBS) for assigned caseload members when applicable and within the employee's role, qualifications, and agency requirements. Documentation must accurately reflect services actually provided.
Transportation Assistance:
Assist the transportation team when needed, including child pickups/drop-offs, supervision during transportation, loading and unloading, route support, and communication of transportation concerns. Employees assigned to drive must follow all agency vehicle and safety policies.
Team Lead Duties, When Applicable:
When assigned as Team Lead, help coordinate daily activities and staff coverage, support transportation and supervision, communicate operational concerns, and order/coordinate food and meals for children.
Additional Duties:
Assist with other reasonable program duties when applicable, including program setup/cleanup, meals, activities, field trips, supplies, incident documentation, staff meetings, training, and general program operations.
WORK SCHEDULE
Monday-Thursday:
Approximately 2:00 PM-7:00/7:30
PM Friday:
As needed based on programming, staffing, transportation, meetings, or other operational needs. Actual hours may vary depending on program needs.
COMPENSATION
$17.00-$21.00 per hour. Starting wage will be based on relevant experience, experience with children, behavioral health or case-management experience, documentation skills, leadership ability, transportation capabilities, reliability, availability, and demonstrated work capabilities.
