Job Description

Overview We are seeking a dynamic and compassionate Resident Care Coordinator to join our dedicated healthcare team. In this vital role, you will serve as the primary point of contact for residents, families, and staff, ensuring the delivery of exceptional care while maintaining compliance with healthcare regulations. Your leadership will foster a supportive environment that promotes resident well-being, safety, and dignity. This position offers an exciting opportunity to make a meaningful difference in the lives of residents across various care settings, including long-term care facilities and assisted living communities. Responsibilities Coordinate comprehensive resident care plans by collaborating with nursing staff, physicians, and families to address individual needs and preferences. Oversee daily resident assessments, vital signs monitoring, and medical documentation to ensure accurate and timely updates in electronic health record (EHR) systems such as Epic or Cerner. Manage medication administration processes Supervise nursing staff and care team members to ensure compliance with nursing policies, long-term care regulations, and safety standards. Facilitate discharge planning, coordinate with case management teams for utilization review and management, and assist with diagnostic evaluations like laboratory tests or medical imaging when necessary. Assist the Administrator on scheduling and schedule coverage. Works the floor 80% and 20 % office or as needed.
