Full Job Description General Purpose :
This position exists to assist the residents and their families with psychosocial needs and discharge planning. The incumbent must perform their responsibilities in accordance with current federal, state, and local standards, guidelines, regulations and protocols that govern our facility. Develop Social Service care plans for each resident and complete assigned sections of the MDS in a timely manner in accordance with regulatory requirements Complete Social History Assessment, Initial Discharge Evaluation, PASSAR validation and Trauma Informed Care questionnaire on admission. Coordinate all aspects of discharge planning to include NOMNC, ABN, associated appeals, Home Health Care referrals, referrals to transitions programs as appropriate and conferences with resident, family, and appropriate staff Assist with arranging post SNF care needs not limited to home health care referrals and DME paperwork Complete and submit Social Security Representative Payee and Veterans Benefits as needed. Coordinate submission of Medicaid applications with resident/family and outside agency/attorney Serve as the Resident Grievance officer and coordinates resolution with identified departments Assist with the

tours, paperwork, demographics, inquiries. Coordinate all aspects of Admission process in the absence of Admission/Marketing Director Attend and participate in resident care conferences Participates as member of Quality Assurance committee Attend, participate and lead continuing education programs designed to keep staff informed of regulatory changes and orient employees to Resident Rights and Prevention of Abuse and Neglect Demonstrate effective record keeping and narrative documentation in alliance with required government forms and facility forms Interact with staff and department heads in a timely manner regarding any needs. Provide counseling services to residents and families within scope of knowledge.
