The Patient Care Assistant is an entry-level position responsible for providing administrative and clinical support within the Memory Clinic. The primary focus of this role is the administration of standardized cognitive assessments and the documentation of Chronic Care Management (CCM) activities within the eClinicalWorks (eCW) platform. This is a non-supervisory role that reports directly to the Clinical Director.

Scope of Services Cognitive Assessments:

Administer standardized cognitive screening tools (e.g., MoCA, SLUMS) to patients as directed.

CCM Documentation:

Record and track monthly Chronic Care Management (CCM) interactions in eClinicalWorks (eCW).

Data Entry:

Input patient information and assessment scores into the electronic health record system accurately.

Patient Scheduling:

Coordinate and confirm appointments for cognitive testing and CCM follow-ups.

Administrative Support:

Assist the clinical team with filing, record-keeping, and preparing patient charts.

Patient Communication:

Communicate with patients regarding program schedules and educational materials. Memory Coach case load management. Census management.

Engagement Requirements Systems:

Proficiency in basic computer applications and ability to learn eClinicalWorks (eCW) for data entry.

Professionalism:

Strong interpersonal skills for working effectively within a healthcare team.

Compliance:

Full adherence to HIPAA regulations and protection of patient health information (PHI).

Timeliness:

Commitment to meeting all documentation deadlines for assessment entry and CCM logging.

Education:

Minimum of a Bachelor's degree in a health-related field (e.g., Psychology, Social Work).

Licensure:
