Dallas Detox, is a proud member of the Quadrant Health Group,is looking for a compassionate and organized Discharge Coordinator who will be responsible for coordinating discharge planning and continuing care services for clients transitioning from residential treatment. This role ensures clients are connected to appropriate post-treatment resources, providers, housing, recovery support services, and ongoing care to promote long-term recovery and stability. The Discharge Coordinator collaborates closely with clinical staff, medical providers, case managers, families, and referral sources to ensure safe and effective discharge planning.
What You'll Do:
Major Tasks, Duties and Responsibilities Develop individualized discharge and aftercare plans for clients prior to discharge Coordinate step-down placements including PHP, IOP, outpatient therapy, sober living, psychiatric services, MAT providers, and community-based support services Schedule follow-up appointments with appropriate providers prior to discharge whenever possible Assist clients with discharge logistics including transportation, medication coordination, housing arrangements, and continuity of care needs Communicate with families, referral sources, and outside providers regarding discharge planning as authorized Verify aftercare appointments, admissions, and placement arrangements Maintain accurate and timely documentation of discharge planning activities in the EMR Collaborate with therapists, case managers, medical staff, and leadership regarding client discharge readiness and barriers to care Educate clients on relapse prevention resources, community recovery supports, and continuing care recommendations Monitor discharge timelines to ensure all required planning is completed prior to discharge Assist with obtaining releases of information and coordinating transfer documentation as needed Ensure compliance with
HIPAA, 42 CFR
Part 2, accreditation standards, and company policies Participate in treatment team meetings and discharge planning discussions Perform other duties as assigned
What You'll Bring:
Skills, Knowledge and Competencies Required Bachelors degree in social work, counseling, psychology, human services, or related field preferred Experience in behavioral health, discharge planning, utilization review, case management, or substance use treatment preferred Knowledge of levels of care within behavioral health and substance use treatment systems Strong organizational and coordination skills with attention to detail Excellent communication and relationship-building skills Ability to work effectively in a fast-paced, multidisciplinary environment Proficiency with EMR systems and Microsoft Office applications Physical Requirements Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed Ability to navigate a residential treatment environment and communicate with clients and staff throughout the facility Work Environment Residential behavioral health treatment setting Frequent coordination with outside providers, agencies, and referral partners May involve interaction with clients experiencing emotional distress, psychiatric symptoms, or substance withdrawal symptoms
