Job Description

Job Summary We are seeking a dynamic and compassionate Resident Care Coordinator to lead and coordinate resident care services within our long-term care or assisted living facility. In this vital role, you will serve as a bridge between residents, families, and healthcare teams, ensuring personalized, high-quality care that promotes dignity, independence, and well-being. Duties Develop and implement individualized care plans based on comprehensive assessments, including vital signs, medical history, and physiological evaluations. Coordinate interdisciplinary team efforts to deliver holistic resident care, ensuring compliance with nursing policies and long-term care regulations. Oversee medication administration, catheter care, tube feeding management, and other clinical interventions following HIPAA guidelines and safety protocols. Conduct regular patient assessments utilizing medical terminology knowledge and diagnostic evaluation techniques to monitor health status and adjust care plans accordingly. Manage documentation of medical records , and ensure accurate recordkeeping using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Supervise nursing staff and care teams, providing guidance on infection control practices, sterile processing procedures, and managing care facility teams effectively. Facilitate discharge planning, family communication, and coordinate with external healthcare providers when needed. Requirements Proven experience working with individuals with disabilities, dementia care experience, or senior care in settings such as nursing homes or assisted living facilities. Knowledge of medical terminology, anatomy knowledge, physiology knowledge, and diagnostic evaluation methods. Demonstrated ability to manage care facility teams with effective managerial communication skills and nurse management experience. Proficiency in EMR systems like Cerner or eClinicalWorks; familiarity with hospital medicine workflows is highly desirable. Valid Basic Life Support (BLS) certification along with skills in, med certification, injection administration, sterile technique, catheterization, specimen collection/processing, and airway management. Ability to perform physical examinations; experience with ventilator management. Knowledge of state healthcare regulations including long-term care regulations and HIPAA compliance standards. Excellent patient assessment skills combined with a compassionate approach to patient care across diverse populations including, geriatrics, behavioral health patients or those requiring memory/cognitive support.
