  • Full-time Job Summary Position Overview Oasis Recovery Center, a licensed and Joint Commission-accredited detoxification and residential treatment facility in Fort Myers, FL, is seeking a dedicated and compassionate Case Manager to join our multidisciplinary treatment team.
The ideal candidate will have experience in substance-use disorder treatment, behavioral health coordination, and discharge planning , with a strong commitment to client advocacy and continuity of care. As a Case Manager, you will serve as the primary liaison between clients, clinical staff, and community resources—helping individuals navigate their recovery journey from admission through discharge. Responsibilities Conduct comprehensive biopsychosocial assessments and assist in developing individualized treatment plans. Facilitate coordination between clinical, medical, and support teams to ensure integrated care. Maintain a manageable caseload and track client progress toward treatment goals. Lead or participate in treatment team meetings , presenting case updates and discharge recommendations. Provide referrals and linkage to community resources including outpatient programs, housing, employment, and aftercare services. Assist clients with aftercare planning, relapse-prevention strategies, and family communication. Maintain accurate and timely clinical documentation in compliance with state, CARF, and facility standards. Educate clients on recovery resources, coping skills, and ongoing treatment options. Uphold confidentiality and ethical standards in accordance with HIPAA and state licensing regulations . Qualifications Bachelor's degree in Social Work, Psychology, Human Services, or related field (Master's preferred). Minimum 1-2 years of experience in substance-use or mental-health treatment settings. Knowledge of ASAM levels of care , detox protocols, and case-management best practices. Familiarity with Florida DCF/Department of Children and Families standards preferred. Strong communication, organizational, and documentation skills. Ability to work collaboratively within a multidisciplinary team environment. Proficiency with EHR/EMR systems (e.g., Kipu, Athena health).
