Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for patients in our inpatient rehab facility. You'll complete assessments, develop individualized discharge plans, and lead communication between patients, families, therapists, nurses, and physicians to support recovery after stroke, brain injury, spinal cord injury, and other complex conditions. Responsibilities Perform comprehensive patient assessments and identify rehabilitation and discharge needs. Develop and manage individualized care and discharge plans for rehab patients. Coordinate interdisciplinary team meetings and communication among clinicians. Educate patients and families on treatment goals, progress, and community resources. Collaborate with payers on authorization, utilization review, and continued stay needs. Arrange post-discharge services, equipment, and follow-up appointments. Advocate for patients to remove barriers to care and safe transitions. Maintain accurate, timely documentation in EMR and ensure regulatory compliance.
