Duties:

• Develop and implement individualized care plans for patients
• Provide counseling and support to patients and their families
• Collaborate with healthcare professionals to coordinate and facilitate patient care
• Assist with discharge planning and ensure smooth transitions for patients ```

Qualifications:

• Associates degree in a related field (e.g., social work, psychology, nursing)
• Previous experience in case management or a related healthcare role preferred
• Strong knowledge of physiology and medical terminology
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced hospital environment
• Strong organizational skills and attention to detail
• Knowledge of patient service principles and practices
