HIM Coordinator Long-Term Care Nursing About the Role Our well-established long-term care nursing facility is seeking a detail-oriented HIM Coordinator to manage resident health information and ensure accurate coding for reimbursement and regulatory compliance. This role is essential to maintaining accurate documentation that supports quality care, proper billing, and survey readiness. Key Responsibilities Maintain and organize resident medical records in accordance with state and federal regulations Ensure timely and accurate completion, filing, and retrieval of records (paper and/or electronic) Review clinical documentation for accuracy, completeness, and compliance Assign appropriate ICD-10 diagnosis codes to support MDS coding, care planning, and billing accuracy Coordinate with the MDS Coordinator and business office to ensure diagnosis codes align with

requirements Conduct regular audits of medical records to identify and correct discrepancies Ensure compliance with HIPAA and resident confidentiality requirements Assist with survey and audit preparation by ensuring records are survey-ready Process record requests from residents, families, physicians, and outside agencies in accordance with facility policy Maintain the medical records retention/destruction schedule per state and federal guidelines Support admissions and discharge processes related to documentation and records transfer Stay current on coding updates, regulatory changes, and documentation best practices Qualifications High school diploma or equivalent required; associate degree in Health Information Management or related field preferred Certification such as RHIT, CCA, or CPC preferred (or willingness to obtain) Prior experience in medical records and/or coding in a skilled nursing or long-term care setting strongly preferred Working knowledge of ICD-10 coding, MDS, and PDPM Familiarity with HIPAA regulations and resident confidentiality standards Strong organizational skills and attention to detail Proficiency with electronic health record (EHR) systems Ability to work independently and collaboratively with clinical and administrative teams What We Offer Competitive salary, commensurate with experience Health/dental/vision insurance
