In this role, you'll coordinate referrals, benefits, and services, conduct outreach and follow-up, and help members navigate medical, behavioral health, and social resources. You will collaborate with nurses, pharmacists, and care managers, document member interactions, track care plans, and escalate clinical concerns. This position requires strong communication, data entry, and organizational skills, plus comfort working with electronic health records and member-facing technologies. Responsibilities Coordinate member care plans, referrals, and services under the direction of clinical staff Conduct outbound and inbound calls to engage members and support care plan adherence Verify benefits and authorize services in alignment with plan guidelines Document all interactions accurately in electronic health records and care management systems Collaborate with nurses, pharmacists, and care managers to close care gaps Identify social determinants of health needs and connect members to appropriate resources Monitor and track appointments, follow-ups, and test results to support continuity of care Protect member privacy and maintain HIPAA compliance at all times Escalate clinical or safety concerns to licensed care team members promptly Required Skills Care coordination Healthcare customer service Electronic health records (EHR) usage Benefits and eligibility verification Referral management Data entry and documentation Call center or outreach operations HIPAA and privacy compliance Scheduling and appointment coordination Basic medical terminologyBenefits
