Bilingual Community Health Worker - Inland Empire & Desert Regions (Travel Role) Heritage Health Network (HHN) Join Heritage Health Network as a Bilingual Community Health Worker (CHW) and help connect Medi-Cal members to care through our Enhanced Care Management (ECM) program. This hybrid role requires 3 days in the field per week at community locations (food banks, shelters, clinics, churches, probation offices, hospitals) and 2-3 days conducting telehealth outreach onsite in Riverside, CA . Spanish fluency is required.
