Bilingual Community Health Worker - Inland Empire & Desert Regions (Travel Role) Heritage Health Network (HHN) Join Heritage Health Network as a Bilingual Community Health Worker (CHW) and help connect Medi-Cal members to care through our Enhanced Care Management (ECM) program. This hybrid role requires 3 days in the field per week at community locations (food banks, shelters, clinics, churches, probation offices, hospitals) and 2-3 days conducting telehealth outreach onsite in Riverside, CA . Spanish fluency is required. Key Responsibilities Conduct telehealth outreach to members and screen for ECM eligibility Submit referrals to health plans and document interactions Engage in field outreach to identify and support potential ECM participants Build relationships with community partners and assist members in accessing healthcare and social resources Requirements High school diploma required; BA or Master's in Public Health, Social Work, or related field preferred Experience in community outreach, social services, or healthcare navigation preferred Valid driver's license, active auto insurance, and reliable vehicle for field work Bilingual in Spanish and English Strong communication, organizational, and time management skills Benefits Health, dental, and vision insurance Paid time off + holidays Competitive pay + quarterly bonuses Professional development and growth opportunities Make a real difference in your community—apply today!
