We provide client-centered services that promote dignity, equity, and well-being. We are seeking a Bilingual Community Health Worker to join our team! About the Role As a Bilingual Community Health Worker (CHW), you will help clients living with HIV stay connected to medical care and access the resources they need to thrive. You'll build trusting relationships, provide education and adherence support, assist with appointment engagement, and help clients navigate healthcare and community resources. You'll communicate with clients in both English and Spanish and provide support in the office, community, and clients' homes. This role also includes front-desk coverage and support. What You'll Do Conduct outreach and case finding to engage or re-engage clients in HIV medical care. Build trusting relationships with clients through consistent, empathetic, and culturally responsive support. Provide individualized education, medication adherence support, and regular client check-ins. Assist clients with accessing medical, social, mental health, housing, transportation, and other supportive services. Accompany clients to medical, legal, social service, and other appointments as appropriate. Collaborate with case managers, clinical staff, and community partners to support continuity of care. Document client interactions, outreach efforts, and services accurately and in accordance with program requirements. Provide front-desk coverage and help create a welcoming, professional environment for clients and visitors. Participate in required trainings, meetings, and program activities. Maintain flexibility to work evenings, weekends, and varied schedules based on program needs What We're Looking For Fluency in both English and Spanish is required to effectively communicate with and support clients. High school diploma/GED required; associate's or bachelor's degree in human services or a related field a plus. 1-2 years of experience in human services or a related field strongly preferred. Strong organizational, problem-solving, and decision-making skills, with the ability to prioritize competing demands and meet deadlines. Knowledge of cultural and lifestyle diversity issues; familiarity with HIV/AIDS and related health and social issues a plus. Working knowledge of local healthcare providers, social service agencies, and public entitlement programs preferred. Strong written and verbal communication skills with the ability to interact effectively with clients, colleagues, and community partners. Demonstrated ability to build effective, respectful, and supportive relationships. Ability to exercise sound judgment and maintain professionalism, integrity, and confidentiality. Proficiency in Microsoft Office (Word, Excel, Outlook) strongly preferred. Valid New Jersey Driver's License and ability to met company driving requirements. Occasional travel will be required, including the operation of the Outreach Van (RV-style) for community-based outreach; regular travel required during the first 3-6 months for training. Employees are expected to participate in and assist with at least two South Jersey AIDS Alliance fundraising events each year, including the NJ AIDS Walk.
