Job Description

Job Overview Join our dynamic team as a Community Health Worker and become a vital link between healthcare providers and the communities we serve! In this energetic role, you will empower individuals by providing health education, support, and outreach to promote healthier lifestyles and improved well-being. Your passion for community engagement and public health will drive meaningful change, especially within underserved populations. This paid position offers an exciting opportunity to make a positive impact while developing your skills in health promotion, program development, and community outreach. Duties Conduct community outreach initiatives to educate residents about health services, preventive care, and wellness programs. Assist clients in navigating healthcare systems, including Medicare and Medicaid enrollment processes. Provide culturally sensitive health education on topics such as nutrition, addiction counseling, and chronic disease management. Collect data through surveys and interviews to evaluate program effectiveness and identify community needs. Support the development and implementation of public health programs tailored to diverse populations. Utilize EMR/EHR systems to document client interactions, service delivery, and follow-up activities accurately. Engage with community organizations to foster partnerships that enhance service delivery and outreach efforts. Requirements Experience working with individuals with disabilities and underserved communities is highly preferred. Strong knowledge of public health principles, law policies, and community engagement strategies. Excellent communication skills, including client communication, public speaking, and motivational interviewing techniques. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) for report writing and data management. Bilingual abilities are a plus to effectively serve diverse populations. Basic life support (BLS) certification or willingness to obtain it upon hire. Familiarity with medical terminology, health education practices, and community health program planning. Experience working with culturally diverse communities using respectful, inclusive approaches.
