Location:
Central Wisconsin (Marathon, Portage, Lincoln, Langlade, Shawano, Wood, Clark, and Taylor Counties) Reports to: Hmong Team Coordinator Full-Time Position - 40 hours/Week Includes in-person events, weekend hours, and some remote work

Nonprofit organization that connects Latino individuals and families to healthcare, education, community services, legal resources, and economic opportunities. The H2N (Hmong and Hispanic Communication Network) project advances health equity for Hmong, Hispanic, and other vulnerable communities. H2N Community Health Workers identify barriers to health and help and bridge gaps between health systems, public health, resource agencies, and the individuals and communities we serve.
Unique Role of Community Health Workers:
Community Health Workers (CHWs) understand the social determinants of health and are responsible for helping clients from under-served communities overcome barriers to health.
Specific Job Responsibilities:
Assist clients with social needs ("case management").
Case management typically involves:
Assessing Hmong client needs and connect them to necessary services and resources Acting as an advocate for clients with health service delivery agencies and personnel. Utilize client management Software to document and manage client cases properly Lead community-based blood pressure outreach/education and screenings in a variety of locations such as farms, churches, community centers, and existing community events. Provide cultural and linguistic outreach, patient navigation, social determinant screening, referrals, and follow-up services to expand community health worker outreach. Serve as trusted extensions of local public health agencies, enabling county health departments to reach rural communities more effectively. Improve health literacy, foster connections to primary care and case managers who can help address social determinants of health that might impede lifestyle changes and medical treatment. Staff community outreach events (weekday, evenings, and weekends) Engage with members of the community at clinics and events, disseminating materials. Attend and participate in weekly H2N team meetings and All-Staff Team meetings. Maintain knowledge of H2N and partner organizations' available services and upcoming events for awareness outreach. Occasional travel to surrounding central Wisconsin areas for events. Marathon, Wood, Portage, Langlade, Clark, Taylor, Shawano, Lincoln Enhancing the organization's reputation by being accountable and professional when engaging in collaboration with other partners If trained, facilitate Healthy Living with Diabetes, LENA Start, Stop the Bleed, and health screenings (Blood Pressure and Vision) or other education classes. Help translate documents and informational brochures and handouts to Hmong Attend networking events to learn about community resources and meet stakeholders Other job-related tasks as assigned Minimum Requirements Age 18+ Preferably fluent in Hmong and English (Writing, Listening, Speaking) Strong interpersonal communication skills Self-motivated with the ability to work independently with minimal supervision Familiar with Google Suite (Drive, Sheets), Video Conferencing (Zoom, Microsoft Teams, Webex) Ability to navigate client service software, Microsoft Suite (Word, Outlook, Excel, PowerPoint) Work effectively and contribute to H2N objectives Reliable transportation Integrity Cultural understanding of Hmong and Hispanic traditions. Complete a Community Health Worker training through an in-state certification program.
Compensation Compensation:
$50,000-$55,000 annually, depending on experience, qualifications, and relevant language skills. The anticipated starting salary for this position is $50,000 annually. Candidates with experience in healthcare, community outreach, case management, program coordination, nonprofit operations, volunteer management, or bilingual/multilingual skills may be considered for compensation higher within the salary range. Employees may participate in HOLA's
SIMPLE IRA
retirement plan with voluntary payroll contributions and a HOLA employer match of up to 3% of compensation. Qualified Small Employer Health Reimbursement Arrangement (QSEHRA): Eligible employees may receive reimbursements for individual health insurance and Medicare premiums of up to $300 per month ($3,600 annually), subject to plan terms and eligibility requirements. Additional details regarding eligibility, plan provisions, and benefits are available upon request and will be provided during the onboarding process.
