Job Description

Position Summary The Director of Nursing (DON) is responsible for the overall leadership, management, and operation of the Nursing Services Department within a Skilled Nursing Facility (SNF). The DON ensures the delivery of high-quality resident-centered care, regulatory compliance, effective staffing, quality improvement, and clinical excellence. This role oversees all nursing personnel and collaborates with interdisciplinary team members to provide safe, compassionate, and effective care to residents. Essential Duties and Responsibilities Clinical Operations Direct, coordinate, and supervise all nursing services within the facility. Ensure residents receive care in accordance with physician orders, care plans, facility policies, and regulatory requirements. Oversee resident assessments, care planning, and clinical documentation. Monitor clinical outcomes and implement interventions to improve quality measures. Ensure timely identification and management of changes in resident conditions. Participate in interdisciplinary care plan meetings and resident reviews. Oversee medication management and treatment administration practices. Regulatory Compliance Maintain compliance with all federal, state, and local regulations governing skilled nursing facilities. Lead facility preparation and response for state surveys, complaint investigations, and CMS inspections. Develop, implement, and update nursing policies and procedures. Monitor documentation standards and ensure clinical record accuracy. Ensure adherence to infection prevention and control standards. Staff Leadership and Development Supervise the Assistant Director of Nursing (ADON), Unit Managers, Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. Schedule, monitor, and adjust staffing to meet resident care needs. Conduct performance evaluations and corrective actions as necessary. Foster a positive work environment focused on accountability, teamwork, and resident-centered care. Develop succession plans and leadership development opportunities. Quality Assurance and Performance Improvement (QAPI) Lead the facility's QAPI program. Analyze quality indicators, survey results, incidents, infections, falls, pressure injuries, and hospital readmissions. Develop and implement action plans to improve resident outcomes. Present quality metrics and performance reports to facility leadership. Infection Prevention and Control Collaborate with the Infection Preventionist to ensure compliance with infection control regulations. Oversee outbreak management, surveillance activities, and staff education initiatives. Monitor antibiotic stewardship efforts and vaccination programs. Risk Management Review and investigate resident incidents, accidents, grievances, and abuse allegations. Ensure timely implementation of corrective actions. Coordinate with administration and interdisciplinary teams to reduce clinical and operational risks. Budget and Resource Management Develop and manage the nursing department budget. Monitor overtime, agency staffing, and labor expenses. Ensure proper management of nursing supplies, equipment, and resources. Identify opportunities to improve efficiency while maintaining quality care. Resident and Family Relations Serve as a clinical resource for residents, families, and staff. Address concerns and complaints promptly and professionally. Promote resident satisfaction and quality of life initiatives. QualificationsEducation Graduate of an accredited school of nursing. Bachelor of Science in Nursing (BSN) preferred. Master's degree in Nursing, Healthcare Administration, or related field preferred. Licensure Current, unrestricted Registered Nurse (RN) license in the state of practice. Experience Minimum of 3 years of progressive nursing leadership experience. At least 2 years of management experience in a Skilled Nursing Facility or Long-Term Care setting preferred. Demonstrated experience with CMS regulations, survey readiness, and quality improvement programs. Knowledge, Skills, and Abilities Thorough knowledge of Skilled Nursing Facility regulations and standards of practice. Strong clinical assessment and decision-making abilities. Knowledge of MDS, reimbursement systems, and Medicare/Medicaid regulations. Experience with QAPI and infection prevention programs. Excellent communication, leadership, and conflict-resolution skills. Proficient in Electronic Medical Records (EMR/EHR) systems. Physical Requirements Ability to stand, walk, bend, reach, and move throughout the facility. Ability to lift, carry, push, or pull up to 25 pounds. Ability to respond promptly to resident emergencies. Working Conditions Skilled Nursing and Rehabilitation Facility environment. Exposure to infectious diseases and other healthcare-related hazards. Participation in on-call rotation and emergency response activities may be required.
