$52,000 - $56,000 (Full Time 35 hours) Thursday - Sunday (10am-6:45pm) Job Overview About us: As a mid-sized nonprofit with a team of more than 50 professionals, Turning Point is proud to provide comprehensive domestic violence services to McHenry County. We are seeking a motivated individual to join our team and make a difference in the lives of individuals and the community. Responsibilities Maintain an assigned caseload of shelter residents and families, providing consistent, individualized, survivor-driven case management throughout their shelter stay. Meet regularly with residents to assess needs, strengths, priorities, barriers, progress, and changing circumstances, and develop and maintain individualized service plans based on resident-identified goals. Provide case management, advocacy, and resource coordination related to housing and housing stability, public benefits, employment and education, healthcare and behavioral health, legal and court-related needs, children's and family needs, transportation, identification and vital documents, safety, and other practical needs. Assist residents with navigating community systems and completing applications, referrals, documentation, appointments, and other steps needed to access resources and achieve identified goals. Maintain active follow-up on referrals, applications, appointments, provider contacts, and identified action steps. Identify barriers, problem-solve with residents, explore alternatives, and adjust service plans as needed. Communicate and coordinate with community providers, systems, and other Turning Point programs, with appropriate releases and in accordance with confidentiality requirements, to reduce gaps and duplication in services. Maintain regular communication with Shelter Advocates regarding residents' goals, needs, progress, barriers, and follow-up. Provide consultation and support to Advocates regarding resources, case-plan activities, boundaries, and appropriate ways to support residents during their shifts. Coordinate discharge and transition planning with residents, including housing, safety, community resources, ongoing Turning Point services, and other identified needs. Provide crisis intervention, emotional support, safety planning, de-escalation, and stabilization as needed. Respond to emerging safety concerns and consult with shelter leadership regarding complex or significant resident concerns. Maintain timely, accurate, and comprehensive documentation of case-management services, resident progress, referrals, contacts, significant changes, and outstanding follow-up needs. Participate in case consultation, supervision, shelter and staff meetings, agency trainings, and other collaborative activities. Assist with shelter screening, admissions, crisis response, helpline services, and other direct shelter responsibilities as needed. Participate in a rotating on-call supervision schedule, including designated weekends, providing after-hours consultation regarding resident needs, crisis situations, and shelter concerns within the scope of the position. Maintain professional boundaries, confidentiality, ethical practice, and survivor-centered, trauma-informed, and empowerment-based approaches in all interactions. Maintain knowledge of domestic violence, trauma-informed services, shelter policies, community resources, and relevant systems. Recommend improvements to shelter services, case-management practices, policies, procedures, and service coordination based on identified resident and program needs. Maintain flexibility to support required trainings, agency meetings, special events, on-call responsibilities, and other organizational or shelter needs outside of the regular schedule as needed. Perform other duties as assigned by the supervisor and/or Turning Point leadership. Qualifications:
- Bachelor's degree in social work, Psychology or Human Service field required. Current CRSS and CPRS in lieu of bachelor's degree may be considered. Master's Degree preferred
- One to two years' experience in domestic violence, human services, crisis work and homeless issues preferred
- Ability to prioritize competing needs, remain organized, and follow through on responsibilities in a fast-paced residential and crisis environment.
- Strong communication, critical-thinking, and problem-solving skills, including the ability to recognize when consultation or additional support is needed
- Obtain ICDVP certification within one year of hire and maintain CDVP certification.
