- Hospice: As a Patient Care Coordinator
- Hospice, yo u will carry out our Mission of Love by planning and managing services for Masonic Village Hospice, focusing on clinical information to assure clinical excellence, optimal clinical outcomes, service excellence, including patient, family, and employee satisfaction, and enhance the financial performance of the operation Our Patient Care Coordinator
- Hospice , is responsible for the following: Maintain knowledge of state licensure, federal certification and accreditation requirements.
Monitor services and documentation, compliance with Medicare Conditions of Participation and compliance with accreditation guidelines and regulations. Assure utilization of agency quality improvement tools. Provide high quality patient care consistent with physician's plan of treatment and hospice care plan. Attend QAPI monthly. Track, trend and prepare reports for Infections, Falls, Oxygen utilization, Wound Care, GI/GU Devices Coordinate all aspects of interdisciplinary hospice care and service delivery. Assisting with patient assignments and scheduling for the clinical team. Assist with patient caseload/visits/admissions as per department needs. Assist with referral process/review to qualify patients for hospice admission. Participate in the Hospice Census review weekly. Review admission and recertification paperwork for accuracy and accountability making sure that guidelines, dates and signatures are in place. Ensure hospice aide supervision visits are performed every two (2) weeks and at least once a year in person for all hospice patients receiving aide visits. Ensure the LPN supervision is completed monthly and at least once a year in person. Ensure on call coverage of patient care on weekends, holidays, and after office hours. Participate in supervisory/nursing on call. Review all hospice documentation paperwork for accuracy and ensure appropriate coordination with contracted vendors. Participate in and conduct Interdisciplinary Team Conference. Coordinate and document all patient hospice clinical patient care services with the hospice team, physician, and patient. Ensure consistency and collaboration with other caregivers. Receive and investigate patient concerns and non-routine events and follow up with Director of Patient Services. Leads and manages the activities of the clinical staff with primary oversight of the Hospice Aide Team. This involves assuring compliance with processes and procedures; evaluating work performance (PAC evaluations); training/clinical competencies and coaching team members on new procedures and providing excellent customer service. Requirements for a Patient Care Coordinator

Hospice:
Graduation from a state-approved practical nursing certificate program. Current licensure as an LPN by the PA State Board of Nursing required. Preferred minimum of three years clinical nursing experience in hospice services and regulatory standards. CPR certification prior to or upon employment. Current knowledge of hospice operational procedures and clinical processes. Must have an understanding of the principles of Federal Conditions of Participation, State and Local Regulations. Attention to detail and high degree of accuracy required. Maintains a current PA driver's license and insurance on personal vehicle. We are proud to be an equal opportunity employer.
