In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Whether assisting with daily activities, managing medical needs, or offering emotional support, you'll be a trusted partner in promoting independence and quality of life for those you serve. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or palliative care. Responsibilities Deliver comprehensive home health care assistance by supporting patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; promptly report any concerns or changes to supervising healthcare professionals. Assist with medication administration following prescribed instructions while maintaining accurate medical documentation in electronic health record (EHR) systems. Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments through memory care techniques and compassionate engagement. Safely transfer patients using proper patient transfer methods and equipment such as Hoyer lifts or gait belts; ensure patient safety during movement and positioning. Maintain patient confidentiality and adhere to all home healthcare regulations and HIPAA guidelines while managing sensitive medical records and documentation. Support end-of-life care needs with dignity and respect, collaborating with hospice teams to ensure comfort and emotional support for patients and families. Qualifications Valid certification as a Home Health Aide (CHHA) issued by the state licensing authority. Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments. Familiarity with EMR/EHR systems for documentation and patient record management. Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR certification. Ability to perform heavy lifting when necessary and utilize patient transfer equipment safely; physically capable of standing, walking, bending, and performing caregiving tasks for extended periods. Strong communication skills for effective client interaction and collaboration with healthcare teams. Understanding of home health care regulations, HIPAA compliance, and infection control procedures.
