Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of patients receiving home and community-based care. In this vital role, you will provide personalized assistance to individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized dementia or hospice care. Your proactive approach and skilled caregiving will promote independence, comfort, and well-being in the familiar environment of their homes. Responsibilities Deliver comprehensive patient observation to monitor health status and report changes promptly. Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support. Administer medications accurately following medical instructions and document medication records diligently. Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track patient health. Support patients with specialized care needs such as Alzheimer's, dementia, memory care, hospice support, and catheter management. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical data securely in compliance with HIPAA regulations. Use assistive devices like Hoyer lifts safely to prevent injury during transfers and mobility assistance. Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and cooking. Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and hygiene. Collaborate effectively with healthcare teams by updating medical documentation and communicating patient progress clearly. Support patients' emotional well-being through compassionate interaction and engagement in social activities. Qualifications Proven experience in home health care or long-term care environments such as nursing homes or assisted living facilities. Familiarity with medical terminology, medication administration procedures, and vital signs monitoring. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or Alzheimer's/dementia training Mental health disability knowledge Experience working with individuals with developmental disabilities or disabilities in general is highly valued. Ability to operate EMR/EHR systems efficiently while maintaining strict confidentiality per HIPAA standards. Strong communication skills for working effectively with patients, families, and healthcare professionals. Physical ability to lift or transfer patients using equipment like Hoyer lifts; stamina for long shifts involving standing or walking.
