Collaborate with family members and healthcare providers to develop care plans that meet individual needs. Skills Proficiency in long-term care practices and home community care techniques. Ability to perform heavy lifting safely and effectively when assisting clients. Strong understanding of resident rights and the importance of dignity in care. Capability to monitor vital signs accurately and respond appropriately to changes in health status. Excellent communication skills, both verbal and written, for effective interaction with clients, families, and healthcare teams.
