Job Overview We are seeking compassionate and dedicated Caregivers/Home Health Aids to provide exceptional in-home and community-based care to individuals of all ages. In this vital role, you will support clients with daily activities, medical needs, and emotional well-being, fostering independence and enhancing quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' conditions, recording vital signs and changes in health status accurately Administer medications following prescribed schedules and document medication administration in medical records or electronic health record (EHR) systems Support clients with specialized needs such as Alzheimer's care, dementia care, hospice care, or working with individuals with developmental disabilities Operate medical equipment like Hoyer lifts, catheters, and other assistive devices safely and effectively Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation Document all patient interactions and medical information in compliance with HIPAA regulations using EMR (Electronic Medical Records) or EHR systems Requirements Prior experience in home health care, assisted living, nursing homes, or long-term care settings preferred Knowledge of medical terminology, medication administration, and patient care procedures Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice or dementia training are a plus Ability to work independently while following detailed care plans and protocols Strong communication skills to collaborate effectively with clients, families, and healthcare teams Experience working with individuals with disabilities or special needs highly valued Valid driver's license and reliable transportation preferred for home visits Join our team to provide compassionate care that truly makes a difference!
