Job Description

Overview Join our compassionate team as a Home Health Aide / Caregiver and make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide essential assistance with daily activities, promote independence, and ensure the safety and well-being of your clients in their homes or care facilities. Your dedication will help enhance quality of life for seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods such as Hoyer lifts when necessary Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA regulations to ensure confidentiality Administer medications following prescribed schedules and medical terminology guidelines, including catheter care and other specialized treatments Support clients with meal preparation, feeding, light housekeeping tasks such as laundry and cleaning to maintain a safe and comfortable environment Provide companionship and emotional support, especially for clients with dementia or Alzheimer's disease, ensuring respectful communication and engagement Assist with medical procedures such as wound care or end-of-life comfort measures under the supervision of healthcare professionals Document patient care activities thoroughly in compliance with home healthcare regulations and maintain accurate medical records for ongoing treatment continuity Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or in assisted living environments Strong knowledge of home health care assistance techniques, including patient transfer methods and medication administration Familiarity with EMR/EHR systems for documentation and medical records management Ability to perform patient observation accurately, recognize changes in condition, and communicate effectively with healthcare teams Knowledge of medical terminology, vital signs measurement, CPR/First Aid certification, and basic life support skills Experience working within home health agencies or hospice care settings is highly desirable Excellent client communication skills coupled with compassionate caregiving abilities to foster trust and rapport Join us to provide dedicated home health care that empowers clients to live safely and comfortably in their own homes. Pay:
$21.00 - $24.00 per hour Expected hours: 4.0 - 40.0 per week
