Facilitate safe patient transfers using manual handling techniques and equipment such as Hoyer lifts, ensuring safety for both clients and caregivers. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning and disinfecting common areas and resident spaces. Provide transportation assistance for appointments or community outings, promoting social engagement and independence. Skills Proven experience working with individuals with disabilities, including developmental disabilities, neurodevelopmental disorders like autism, or dementia-related conditions such as Alzheimer's and other memory care needs. Strong knowledge of patient observation techniques, patient monitoring, and recognizing signs of distress or health deterioration. Familiarity with care plans development and implementation to ensure personalized support strategies are followed effectively. Competence in basic nursing skills including vital signs measurement, medication administration, first aid, CPR certification, and emergency response procedures. Ability to perform heavy lifting and manual handling safely while adhering to safety protocols to prevent injury. Excellent communication skills for engaging with clients respectfully while supporting their resident's rights and fostering trustful relationships. Experience working within home health settings or long-term care environments such as nursing homes or hospice care is highly valued.
