Organize and develop daily tasks in accordance with the individual's ISP outcomes and strategies. Facilitate appropriate programming in areas of habilitation activities, prevocational training and community inclusion. Familiarize yourself with assigned individuals ISP outcomes and strategies, and record the individual's daily progress towards those outcomes in the OTC's electronic record keeping system (Therap) in an appropriate and timely manner. Provide responsible community based activities to provide consumers with opportunities to develop interpersonal and social awareness skills. Maintain an awareness of the needs of the individuals with disabilities, and immediately report any related concerns or emergencies directly to the supervisor. Assist individuals served with daily needs including assisting individuals with feeding and changing needs. Responsibility includes full compliance with Danielle's Law and Stephen Komninos' Law. Transport individuals with disabilities to and from residence and various community outings, which may require beginning the work shift early or ending the work shift late. Willingly and effectively cooperates with OTC, The Department of Human Services, The Division of Developmental Disabilities (DDD), and other licensing or state agency or local municipalities in any inspections and investigations, upon request. Other duties as assigned. Driving individuals to and from their homes and community activities. Responsible for administering prescribed medications to individuals served. Driver's License valid in the state of NJ with a driving record acceptable to OTC Services and Motor Vehicle Commission (MVC) Regulations. Must be driving for at least 3 years and have a full (not provisional or probationary) license. Must be 21 years of age. Must be able to effectively communicate both verbally and written. Ability to bend, lift and assist in physical transfer of individuals needing assistance. Ability to drive large vehicles including non-ambulatory lift vehicles.
