Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' health by taking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration following care plans and ensuring compliance with HIPAA regulations to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Provide transportation assistance for appointments, errands, or community outings to promote social engagement and independence. Document care activities diligently, update care plans regularly, and communicate effectively with clients, families, and healthcare teams about resident's rights and progress. Qualifications Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients closely and respond appropriately to changes in condition or behavior. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism is advantageous. Certification or training in CPR and First Aid is required; ongoing education in patient monitoring and HIPAA compliance is a plus. Experience with manual handling equipment like Hoyer lifts or patient transfer methods is desirable. Strong communication skills for effective client interaction and team collaboration.
