Job Description

Job Overview Join our compassionate team as a Home Health Aide, where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. You will work closely with patients, families, and healthcare professionals to provide comprehensive support that enhances quality of life and fosters well-being. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by observing vital signs and reporting changes promptly to healthcare providers Administer medications accurately following prescribed instructions while maintaining detailed medical documentation Support patients with specialized needs including dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities or disabilities Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and meal preparation Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical records securely in compliance with HIPAA regulations Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation Requirements Proven experience in home health care, nursing homes, assisted living facilities or long-term care environments Knowledge of medical terminology and procedures related to patient care Valid CPR certification and training in catheter care, hospice care, dementia care, and medication administration preferred Ability to work independently while following healthcare plans and protocols precisely Strong observational skills for patient safety monitoring including assessing vital signs and recognizing changes in condition Experience working with individuals with disabilities or developmental challenges is highly valued Familiarity with EMR/EHR systems and medical documentation standards Compassionate attitude combined with excellent communication skills and professionalism Ability to lift or transfer patients safely using equipment like Hoyer lifts when necessary Join us in providing exceptional home health services that truly make a difference!
