Homemaker / Home Health Aide / Personal Care Assistant Home in the Heart Healthcare Agency Milwaukee, WI Job Details $17 an hour 1 day ago Qualifications Care plan implementation Full Job Description The Homemaker / Home Health Aide / Personal Care Assistant provides non-medical, in-home support to clients who need assistance with daily living activities. The caregiver helps clients remain safe, comfortable, independent, and well cared for in their homes while maintaining dignity, respect, and confidentiality. Responsibilities Assist clients with bathing, grooming, dressing, and personal hygiene. Assist with toileting and incontinence care when needed. Help clients with transferring, walking, positioning, and mobility. Assist with eating and meal preparation. Prepare light meals according to the client's preferences and care plan. Provide medication reminders only as permitted by agency policy. Perform light housekeeping, including: Laundry Washing dishes Changing bed linens Sweeping and light mopping Cleaning the client's immediate living areas Taking out household trash Provide companionship and emotional support. Assist with errands, grocery shopping, and appointments when authorized. Observe the client's condition and immediately report changes or concerns to the supervisor. Follow the client's individualized care plan. Document services provided accurately and timely. Clock in and out using the agency's approved EVV/timekeeping system. Maintain client confidentiality and comply with HIPAA requirements. Follow infection-control, safety, emergency, and fall-prevention procedures. Maintain professional boundaries with clients and family members. Notify the agency promptly of missed visits, schedule changes, emergencies, or concerns. Perform other non-medical personal care and homemaker duties as assigned within the caregiver's scope of practice. Qualifications High school diploma or GED preferred. Previous caregiving, homemaker, home health aide, CNA, or personal care experience preferred. Must be compassionate, dependable, patient, and professional. Must be able to communicate effectively with clients, families, and agency staff. Must be able to follow written care plans and agency policies. Must successfully complete required background screenings and employment requirements. Reliable transportation is required when traveling to client homes. Valid driver's license and automobile insurance may be required for assignments involving transportation. CPR/First Aid certification preferred or required based on agency policy. Physical Requirements The employee must be able to perform the physical duties necessary to safely assist clients, which may include standing, walking, bending, reaching, pushing, pulling, and assisting with transfers or mobility. The employee must be able to safely perform all assigned duties according to the client's care plan and agency policies. Scope of Services This is a non-medical personal care position. The caregiver does not provide skilled nursing services, administer medications, perform wound care, give injections, or perform other clinical services that require a licensed healthcare professional unless separately licensed and specifically authorized to do so. Expectations All caregivers are expected to provide services with compassion, dignity, respect, professionalism, and reliability while representing the agency appropriately in every client's home.
