Job Summary Join our dedicated healthcare team as a Home Health Aide and make a meaningful difference in the lives of patients by providing compassionate, personalized care in their homes. In this vital role, you will assist individuals with daily living activities, monitor health conditions, and support their overall well-being. This position offers an opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. •Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support to patients. Assist with activities of daily living (ADLs) such as toileting, feeding, and medication reminders. Monitor and record vital signs like blood pressure, pulse, temperature, and respiration rates using medical terminology accurately. Support patients with specialized needs such as catheter care, dementia or Alzheimer's care, and end-of-life comfort measures. Utilize EMR (Electronic Medical Records) systems to document patient information securely while maintaining HIPAA compliance and patient confidentiality. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment. Employ patient transfer methods safely using equipment like Hoyer lifts or other mobility aids; follow proper patient transfer techniques. Recognize signs of medical emergencies and respond promptly with first aid or basic life support (BLS), including CPR when necessary. Communicate effectively with clients and family members to ensure understanding of care plans and address concerns compassionately. Adhere to home healthcare regulations and standards while providing consistent, high-quality care tailored to each individual's needs. Requirements Proven experience working as a Home Health Aide or in assisted living/nursing home environments preferred; experience working with individuals with developmental disabilities or geriatrics is highly valued. Knowledge of medical terminology, medication administration procedures, and vital signs monitoring. Strong communication skills to effectively interact with patients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines. Ability to work independently in a home setting while following detailed care plans; flexibility to accommodate live-in employment or varying schedules is advantageous.
