Job Description

Direct Support Professional -- Clearwater Assisted Living, LLC is a rapidly growing assisted living organization with core competencies in assisted and transitional care in the following Wisconsin communities: Roberts, River Falls, New Richmond, Baldwin, Ellsworth, Hammond, Osceola, Hudson, Somerset, Bay City, and Prescott. Our primary staff are Direct Support Professionals who assist our clients in-home offering assistive services.
General Role Description:
We're currently looking for a Direct Support Professional to provide personal care assistance for our clients. The ideal candidate should be independent, empathetic and thorough on the job with a strong commitment to helping others. This position will be part time with the possibility of full time in the future.
Responsibilities:
Assist people with disabilities to manager their everyday life - scheduling appointments, keeping their home clean, hygiene reminders, medication management, meeting independent living goals Consistently meet the expectations of completing client logs daily and reporting accurate mileage logs Follow the Individual Care Plan (ICP) for each client Uphold a high standard of care by showing up to work each day on-time with a positive attitude and integrity to work with your clients in a diligent manner Follow our scope of services and provide assistance as needed for encouraging and reminding clients to complete activities of daily living
Skills:
Ability to think ahead and be proactive - problem solve as client challenges arise Work independently with a population of individuals with physical and developmental disabilities Ability to follow detailed training instructions on personal cares
