Job Description

Deliver vital in-home care as a Licensed Practical Nurse, applying your skills to support geriatric patients in a compassionate, independent setting. This contract opportunity centers on home health services and requires candidates who are experienced, detail-oriented, and committed to providing quality care in the Omaha, AR area.
Schedule/Hours:
Flexible shifts depending on patient needs
Essential Qualifications:
Active Licensed Practical Nurse (LPN) license At least one year of recent home health experience Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR Excellent clinical assessment and detailed documentation capabilities Reliable transportation to conduct home visits Strong ability to manage caseloads efficiently and independently
Key Responsibilities:
Deliver skilled nursing care according to individualized plan of care in patient homes Monitor patient progress, update care plans, and adjust nursing interventions as needed Administer medications and perform prescribed medical treatments following state regulations Educate patients and their families on care management, disease prevention, and overall wellness strategies Accurately maintain clinical records using HCHB EMR, ensuring compliance and quality documentation Collaborate with registered nurses, therapists, and other healthcare professionals for coordinated patient care
