  • Shift Details:
  • Day shift from 0600-1400
  • Licensed Practical Nurse (LPN)
Duties & Responsibilities:
  • + Provide direct patient care under the supervision of a Registered Nurse or physician.
+ Administer medications and treatments as prescribed. + Monitor and record patients' vital signs and report changes in condition. + Assist with patient assessments and implement individualized care plans. + Perform wound care, specimen collection, and other basic nursing procedures. + Document patient care accurately in the electronic medical record (EMR). + Educate patients and families on health maintenance and treatment plans. + Assist with admissions, discharges, and transfers. + Maintain a safe, clean, and therapeutic environment for patients. + Follow facility policies, infection control protocols, and regulatory standards.
  • Minimum Qualifications
  • + Current, valid, and unrestrictedCA license/certification in good standing, as required for the position.
+ Minimum of 1 yearof recent experience as a Licensed Practical Nurse. + Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required. + Copy of all current required certifications and compliance documents (e.g., .
BLS, N95
fit test, immunization records, TB test, or other client-specific requirements).
