In this vital role, you will provide high-quality nursing care directly in patients' homes, promoting health, safety, and independence. Your expertise will help deliver personalized care plans, monitor patient progress, and support families through complex health needs. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation. Administer medications, injections, IV infusions, and other treatments following established protocols and physician orders. Manage wound care, catheterization, tube feeding, and ventilator support as needed for patients with complex conditions. Document all patient interactions accurately using Electronic Health Record (EHR) systems such as Epic or eClinicalWorks, ensuring compliance with HIPAA and confidentiality policies. Educate patients and caregivers on disease management, medication administration, nutrition, and safety measures tailored to individual needs such as Alzheimer's care or dementia management. Coordinate with interdisciplinary teams for case management, diagnostic evaluations, and discharge planning to ensure seamless patient care. Respond promptly to triage calls and urgent situations, providing critical advice for acute pain management or infectious disease concerns. Requirements Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience working in home health settings or similar environments such as skilled nursing facilities or assisted living communities. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and long-term care regulations. Experience with EMR/EHR systems like Cerner or Home & Community Care software is highly preferred. Demonstrated skills in patient assessment, medication administration—including injections—and sterile technique. Critical care experience or ICU background is advantageous; familiarity with trauma centers or emergency medicine enhances your versatility. Ability to work independently while maintaining excellent caregiver communication and teamwork skills. Certifications in Basic Life Support (BLS), First Aid, and Aseptic Technique are required; additional training in IV infusion or hospice care is a plus.
