POSITION SUMMARY
Reliant Home Healthcare Services is seeking compassionate, dependable, and clinically skilled Licensed Vocational Nurses (LVNs) to join our home healthcare team. We have multiple positions available throughout Riverside County The LVN will provide skilled nursing care to patients in their homes while working collaboratively with patients, families, physicians, registered nurses, caregivers, and the Reliant Home Healthcare Services clinical team. The ideal candidate is professional, reliable, compassionate, attentive to detail, and committed to providing safe, high-quality patient-centered care in the home setting.
ESSENTIAL RESPONSIBILITIES
The LVN will be responsible for providing nursing services according to the patient's plan of care and applicable company policies and requirements.
Duties may include:
Provide skilled nursing care to patients in their homes. Perform patient assessments and monitor changes in the patient's condition. Monitor and document vital signs and other clinical observations. Administer medications and treatments as ordered and within the LVN's scope of practice. Assist with medication management and medication reconciliation as assigned. Provide wound care and other nursing treatments as ordered. Monitor patients for changes in health status and promptly report significant changes to the appropriate supervisor or healthcare provider. Follow the physician's orders and individualized plan of care. Provide patient and family education regarding medications, treatments, safety, and disease management. Assist patients with activities and needs as appropriate to the patient's care plan. Maintain infection-control and safety practices. Communicate effectively with patients, family members, caregivers, physicians, and the Reliant clinical team. Complete accurate and timely nursing notes and required clinical documentation. Document services provided during each visit or shift according to company requirements. Complete required electronic documentation, timekeeping, and visit verification. Maintain patient confidentiality and comply with HIPAA requirements. Report incidents, changes in condition, medication concerns, falls, or other significant events according to company policy. Participate in care coordination with the interdisciplinary healthcare team. Follow all Reliant Home Healthcare Services policies, procedures, and clinical standards. Maintain professional boundaries and provide respectful, compassionate care. Perform other nursing duties within the LVN's scope of practice as assigned.
QUALIFICATIONS
Applicants must: Hold a current and unrestricted California Licensed Vocational Nurse (LVN) license . Maintain current CPR/BLS certification as required by the company and applicable assignment. Meet applicable home healthcare employment and health-screening requirements. Demonstrate strong communication and interpersonal skills. Be dependable, punctual, and professional. Demonstrate the ability to work independently in a home healthcare environment. Have strong clinical documentation skills. Be comfortable using electronic charting and timekeeping systems. Be able to communicate effectively with patients, families, and members of the healthcare team. Demonstrate compassion, patience, professionalism, and respect for patients and families.
ORK ENVIRONMENT
This position is primarily performed in patients' homes throughout San Bernardino County and Riverside County . LVNs may be assigned to different patients and locations depending on patient needs, staffing requirements, availability, and qualifications. Because home healthcare requires independent clinical practice, nurses should be comfortable working in a patient's home while maintaining communication with the Reliant Home Healthcare Services clinical and administrative team.
