The LVN/LPT will be responsible for monitoring the care and health of clients, assisting with day-to-day nursing care, and maintaining and documenting patient records.

Responsibilities and Duties:

Administer prescribed medications and g-tube feedingCollaborate with registered nurses for client care plansTake doctor's orders over the phoneEducate and update staff on medical care plansTrain and work with CNAs and direct care staff
