As a Health Services Coordinator (LPN), youll play a hands-on role in improving the lives of our residents by: Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
