$32.00-$40.00 per hour, based on experience Must hold an active RN or LPN license in the State of Illinois.

Work Location:

Stonebridge Nursing and Rehabilitation This is an in-person position. Duties Provide compassionate and professional nursing care to residents in accordance with individualized care plans. Assess and monitor residents and promptly report changes in condition to the appropriate healthcare provider. Administer medications and treatments as ordered and within the nurse's scope of practice. Accurately document resident care, treatments, medications, observations, and changes in condition. Monitor residents' overall health and communicate concerns to physicians, nursing leadership, and other members of the interdisciplinary team. Follow physician orders and ensure treatments and nursing interventions are completed appropriately. Assist with the development, implementation, and ongoing evaluation of resident care plans. Provide direction and support to Certified Nursing Assistants (CNAs) and other nursing staff as appropriate. Communicate professionally and compassionately with residents, families, Responsible Parties, physicians, and team members. Respond appropriately to changes in resident condition and emergency situations. Follow infection prevention and control procedures and promote a safe environment for residents and staff. Maintain accurate and timely clinical documentation. Participate in shift reports and communicate important resident information to incoming and outgoing nursing staff. Maintain resident dignity, privacy, confidentiality, and HIPAA compliance. Follow all facility policies, procedures, and applicable nursing standards and regulations. Work collaboratively with the interdisciplinary team to support positive resident outcomes. Performs other duties as assigned. Qualifications Current and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois is required. Previous experience in skilled nursing, long-term care, rehabilitation, senior living, or a related healthcare setting is preferred. New graduates who meet licensing requirements are encouraged to apply. Strong clinical assessment and critical-thinking skills. Excellent communication and interpersonal skills. Ability to work effectively with residents, families, physicians, CNAs, nursing leadership, and other members of the healthcare team. Strong organizational and time-management skills. Ability to manage multiple resident needs and priorities during a shift. Experience with electronic healthcare documentation systems is preferred. PCC experience is a plus.
